Community Transitional Support

Supporting your goals to live in the community by facilitating the moving out process, including finding housing, identifying roommates (if desired), developing schedules, and finding community connections. 

 Home-Based Support
Self-Direction Assistance

Planning and administration supports if you receive state-funded home-based services, such as PSW searches and enrollments, budgeting, resource connections, and other personalized services. 

 Personal Development Classes

Sessions in group settings covering a variety of topics, all focused on maximizing your potential and achieving your goals, while building additional skills along the way.
